How do I purchase materials and/or services with a purchase order?

Purchasing Materials

  • Download and complete our order form.

  • Complete either a check or a school/school district district purchase order form.

  • Submit both documents to us using one of the following methods:

  • Email: orders@95percentgroup.com

  • US Postal Service: Tools 4 Reading | 475 Half Day Road, Ste 350 | Lincolnshire, IL 60069.


Purchasing Professional Development Services

  • Complete a school/school district purchase order form. List professional development services as line items on the form.

  • For professional development service line items, please detail the name of the service(s) you are purchasing and the contact information for all participants (first name, last name, email address) according to the following process:

  • If you are registering fewer than 10 participants, please include the participant contact information in the description on the purchase order.

  • If you are registering 10 or more participants, please append a spreadsheet in either .xlsx or .csv format with the participant contact information.

  • Submit the purchase order form to orders@95percentgroup.com.

If you are submitting a purchase order for Sound Wall Classes, please include the following in the description field:

  • First and Last Name of the Participant(s)

  • Email Address for the Participant(s)

  • Date(s) and time (if applicable) for the Sound Wall Class

Reminder: You must purchase a seat for each participant that you intend to attend Sound Wall Classes.  We will only issue certificates of completion to participants who have a registration.


Purchasing a Combination of Materials and Professional Development Services

To submit a purchase order for a mixture of materials and professional development services, please do the following:

  • Complete a school/school district purchase order form.

  • Please include all materials and services to be purchased as line items on the order form.

  • For professional development service line items, please detail the name of the service(s) you are purchasing and the contact information for all participants (first name, last name, email address) according to the following process:

  • If you are registering fewer than 10 participants, please include the participant contact information in the description on the purchase order.

  • If you are registering 10 or more participants, please append a spreadsheet in either .xlsx or .csv format with the participant contact information.

  • Submit the purchase order form to orders@95percentgroup.com.
